CASE
A 78-year-old white male presented for assessment of a lesion located on the right side of the neck. The lesion, which appeared 2 years prior, had since increased steadily in size, spread horizontally, and grown increasingly red in color. The patient relieved intermittent pruritus with OTC moisturizers. Minor trauma to the area caused occasional bleeding. His history included sporadic sun exposure and a partner who smoked cigarettes. On examination, a 6.134.2-cm shiny, erythematous patch with well-defined but irregular borders was present on the right upper lateral aspect of the neck and both the preauricular and postauricular areas of the right ear lobe (Figure 1). Photodamage was present on the face and hands. Other findings were noncontributory.
